The Village of Northbrook is expanding eligibility for the Northbrook Vaccination Clinic. The following groups (Northbrook residents only) are eligible for an appointment:

  • Residents age 65+
  • Residents in Phase 1a, 1b, or 1b+
  • Residents in the following professions:
    • Higher education staff, government workers, media, restaurant staff, construction trade workers, and religious leaders

The clinic is taking place by appointment only on Thursday, April 1 (Pfizer first dose) and Thursday, April 22 (Pfizer second dose) at 1657 Shermer Road (formerly Grainger). Residents signing up to be vaccinated must be available on both April 1 and April 22.

The call center is open now and will operate until 4pm or until all appointments are filled. Appointments are still available and call wait times are down. Residents may call 847-461-8130 to make an appointment. An email address will be needed to register for an appointment. Residents will receive a confirmation email with a consent form that they must fill out and bring to the appointment.

There are approximately 1,200 vaccine doses available, and appointments will be made on a first come, first served basis. Appointments are available from 11am-5pm. Residents must have an appointment to enter the clinic and must bring a valid state ID/driver’s license with a Northbrook address, the consent form, and an insurance/Medicare card.

1. Who is eligible for this vaccine clinic?

The following groups (Northbrook residents only) are eligible for an appointment:

  • Residents age 65+
  • Residents in Phase 1a, 1b, or 1b+
  • Residents in the following professions:
    • Higher education staff, government workers, media, restaurant staff, construction trade workers, and religious leaders

Phase 1b+ includes individuals age 16 and older with the following conditions: cancer, chronic kidney disease, Chronic Obstructive Pulmonary Disease (COPD), diabetes, heart conditions, immunosuppressed states from a solid organ transplant, smoking, obesity, pregnancy, pulmonary disease, and sickle cell disease.

2. How do I make an appointment?Residents may call 847-461-8130 tomorrow (Tuesday, March 30) from 10am to 4pm (or until all appointments are filled). Residents cannot come to Village Hall to make an appointment, and there is no online registration option. There is no voicemail or callback option for residents who cannot wait on hold. When all appointments are filled, callers will hear an automated message stating such.

An email address will be needed to register for an appointment. You will receive a confirmation email with a consent form that you must fill out and bring to the appointment.

3. Does it cost anything? There is no cost. Jewel-Osco will ask for a copy of your state ID/driver’s license and insurance/Medicare card to charge your insurance an administrative fee, as allowed by the Illinois Department of Public Health.

4. What do I need for my appointment?Bring an insurance/Medicare card (if you have one), the consent form you received via email, and a valid state ID/driver’s license with a Northbrook address. You must be able to provide your own transportation to and from the clinic. It is unknown how long an individual appointment could take, but residents can expect a total time of 45-60 minutes from registration to exit.

5. Do you have someone available to help with transportation? Residents must arrange their own transportation to and from the clinic. The Village Church of Northbrook is offering their services to the community and has volunteers available to provide transportation. Contact Ron at ron.mantegna@comcast.net.

6. Where do I go for my appointment?The clinic is being hosted at 1657 Shermer Road (formerly Grainger). Vehicles should enter the parking lot from the south entrance. See a traffic flow map here. Masks and six-foot social distancing will be required inside the clinic. The clinic is handicap accessible.

7. Which vaccine is being offered?Pfizer. The first dose is on Thursday, April 1, and the second dose is on Thursday, April 22.

8. The second dose appointment date/time doesn’t work for me, can I still register?Residents signing up to be vaccinated must be available on both April 1 and April 22. Please do not register if you cannot attend both appointments.

9. Is there medical staff on site in case I have a negative side effect? There will be medical staff on site in the observation area to monitor patients for any potential side effects.
